  Use this form to request a payment for Club Line support.

in order to receive payment as a club line, the club line must submit the form below;
  the referee does not collect this information, they only note on the game card your
  name for payment verification.

complete all the information requested in the form; all fields are required to have an
entry; use "na" (not applicable) in fields marked with an "!"
Club Line Payment form click on "submit" to send.
Club Line Payment form after you submit the report, print a copy of the "Club Line Payment Form Submittal
Page" for your records.
Club Line Payment form payment requests submitted within 24 hours of the last game day in the month will be
processed with the referee payments for that month; if this deadline is missed the
payment will occur the following month.
Club Line Payment form the SSN is required for payment to individuals.
Club Line Payment form if you want the payment to go to a team or club, the non-profit number of the team or
club must be included in the form.
Club Line Payment form the payment amount is the same as that for Assistant Referees;
go to "Club Line Payment Schedule" to see the amount.
