24 Hour Notice Form
Use this form to report the following events within 24 hours:
any incident of poor conduct or abusive behavior by a player, coach or spectator
a "no-show" by an assigned referee
any problem or discrepancy with a field (i.e., field not lined; goals not set-up;
no corner flags; dangerous field situations, etc.)
referee forgot to return the passes to the coach! (auwe!! - referee must make
arrangements to contact the coach and return the passes to the coach!)
any other problem requiring league attention within 24 hours
after you submit the form, print a copy for your records
after submittal you will receive an email confirmation that the 24 Hour Notice has been
received; if you do not receive an email confirmation receipt in 48 hrs. send an email
to Assignor@OahuLeague.com listing the information you previously sent.
