		Maintenance: the Scheduler and Assignor need to update the schedules and assignments for Sunday on Saturday night; in order to do that website maintenance will be done between 9:00 pm and 10:00 pm on Saturday night; at these times if you try to submit an on-line game report or access the database you will receive an error message; submit the on-line report again after maintenance is completed; mahalo for your understanding and cooperation!
		use this on-line form to report the 1) game score, 2) any coach and/or player misconducts, 3) any coach and/or spectator comments and 4) any other incidents of the game within 24 hours; remember to mail the paper game card to arrive at the OL by Tuesday!
		Required fields: the input fields for the referee, game number; home team number and score; and visitor team number and score are required to be completed or the report will not be processed; the information to complete this form can be found on the paper game card or the web schedule; input that information into this on-line form before you mail the paper game card! remember for the score to register, the four digit team no. must be entered, i.e., 0909 or 1854; do not enter letters, spaces or any punctutation marks or symbols; do not enter the team name; only enter the four digit team number! make sure that the team numbers for home and visitors are not interchanged!





		Misconducts: if you submit a misconduct report you need to report the player's name and number or the coach's name; if you need to report more than five misconducts for a team you will need to file a second report!
		the other field entries for player and coach misconducts and coach and spectator comments are not required and may be left blank when there is nothing to report.
