Send Off Procedures and Report

Oahu League (OL) referees are to follow the procedures detailed below when filing a “Send Off” report (ejection or red card) for players or coaches.
First, at the field, the player pass must be collected from the coach and included with the game card and a copy of the “Send Off Report” when it is mailed to the OL at the UPS Store (Oahu League, 4725 Bougainville Drive, PMB #538, Honolulu, HI 96818-3179). The game card and pass must be delivered to the OL at the UPS Store within 48 hours (i.e., no later than Tuesday following a Sunday game); write "Red Card Inside " on the envelope.
 
Send the following to the Oahu League by US mail:
 
for a player: 1) game card; 2) player pass, and; 3) send off report.
 
for a coach: 1) game card, and; 2) send off report.
 
Note: do not attempt to collect the Coach pass and lanyard which is around the coach’s neck.  When the Assignor receives the Send Off Report indicating that the the coach has received a send off, the referee for the next game will be notified that the coach is not eligible to participate as a coach for the duration of the sanction (remember, "no card, no participation")
  A separate report must be submitted for each player or coach sent off for misconduct.
Second, the on-line "Send Off Report" below must be completed and sent to the Assignor immediately after the match.  Each item in the report must be completed.
In describing the misconduct the referee must use only the seven offenses listed on the sendoff report.  The referee must be precise, discriminating and selective in the wording and description of the misconduct as this information is used by the OL Disciplinary Committee to determine the sanctions the player or coach will receive.
The prescribed sanctions for misconduct which the OL uses in determining sanctions are described in "Rule 3.5 - Misconduct by Player" of the Hawaii Youth Soccer Association Team Manual (starting on p. 30); the text of Rule 3.5 is linked below. Remember to print two copies of the report after you submit it - mail one copy with the game card and pass, and keep one copy for your records.

after you submit the report, print two copies of the "Send Off Report Submittal Page"; mail one copy with the game report; keep one copy for your records.
after submittal you will receive an email confirmation that the Send Off Report has been received; if you do not receive an email confirmation receipt in 48 hrs. send an email to Assignor@OahuLeague.com listing the information you previously sent.
